ENROLLMENT APPLICATION 2025-26
You must complete and submit a separate application for each child you are applying to enroll. Please fill in all information on the application below. WPA does not discriminate in enrollment. Make sure you can be reached at the phone number(s) and email address you provide on the form. When or if you are notified that a space is available for your child, you must submit additional information (shot records, birth certificate, signatures) within 7 days or your spot may be given away.
KINDERGARTEN APPLICANTS MUST BE 5 YEARS OLD ON OR BEFORE AUGUST 31, 2025.
